Celtic-Rangers, Millwall-West Ham, Gremio-Internacional and Boca Juniors-River Plate.
Barca vs Real isn´t a derby, they´re just rivals. :) Definately ManU vs Man City for me, with Chelsea - Arsenal placing second.

If we see how many trophies had AC Milan and inter together ( 18+18 ) in Seria A and 7+3 in Champions League that will be definitely biggest derby in the world.
But if we see numbers of kills or something, that will be Fenerbahce-Galatasaray.
I like derbies where both teams played in same stadium. :)
And please dont talk here about barca-real like derby !

Not same city. Fans dont go in away matches, that is enough why that not derby.

I think it´s Atletico vs Real in Spain for me, because of the stature of the clubs. Don´t know much about the rivalry though. Inter and AC Milan isn´t the biggest one for me because Inter has shown horrendous results in the past two seasons, since Mourinho has left for Real. City and United are two big clubs in England, one club has the biggest stars in the club and the other has the biggest stature in England.

Before 10 years City was played in 3rd divions in England.
And last year won FA Cup first trophy after 1963.

Well Mu-Arsenal is also considered a derby, but not like Spurs vs Gunners... the great north london derby !:)
As Roma - Lazio is the best derby about the supporters, there is not doubt!
